About Us
Access to Care is a primary health care program for low-income, uninsured and underinsured residents of suburban Cook County and northwest Chicago. Eligibility guidelines: Family income under 300% of federal poverty level, live in suburban Cook County or northwest 'Chicago and be ineligible for Medicaid, CountyCare, or Medicare.
